Transitional Case Management

Transitional Case Management (TCM) is a community-based service, short-term program designed for individuals struggling with mental health and/or addiction concerns who would benefit from support to address their immediate needs and to enhance overall mental health, safety, and well-being. Transitional Case Management Services are voluntary and provided within a framework of psychosocial rehabilitation and recovery that promotes self-determination, hope, and improved quality of life

Goals & Objectives

This program is designed to help you navigate community supports, connect to resources and services and meet your short-term goals. A TCM might help you address housing issues, help transition back to community from hospital for example, budgeting, finding social and recreational supports and supporting you in making sure your physical and mental health needs are being addressed. The goal is to support the stabilization of clients in their natural environments by building healthy coping skills to live a fulfilling life.
