Substance Use Outreach
The purpose of the Substance Use Outreach Program is to provide clients with community treatment for addictions concerns, in a mutually agreed upon location, that is comfortable and easily accessible for the client.
Substance Use Outreach Counsellors will primarily provide counselling and treatment to support clients in minimizing or eliminating barrier(s) to attending services at 200 Queens Ave, London. Clients of this program participate in Coordinated Care Plans as needed, and staff assess the need for additional referrals for supports to ensure wrap-around care.
Goals & Objectives
This program is designed to improve the health, social outcomes and quality of life of people with substance use concerns through offering direct client services while minimizing barriers to treatment.
A barrier may be related to an individual’s physical health or mobility issues, mental health, the need for anonymity or because of a concern related to the individual’s personal safety. Staff within the SU outreach program will collaborate with individuals within the program to determine an alternate meeting location that is confidential and safe.
This program operates from a client-centered approach and recognizes all treatment goals (abstinence, harm reduction, moderation). This program is designed to increase the accessibility of services with substance use concerns with a priority on youth, older adults and women at risk of violence, while improving linkages between community partners.

Family and Significant Other Intake Form
Our Family Group provides family members with a forum in which to discuss successes and challenges in coping with their loved one’s addiction, healthy ways to support your loved one, and how to navigate through the addiction treatment system.
Family Group is held on the 3rd and 4th Monday of each month, from 5:30-7:30 pm.

How Do I Get An Appointment?
Anyone concerned about their own addiction can access screening, assessment and treatment services. These services are available for all individuals regardless of their stage of change or treatment goal. Third party and self-referrals are welcome, but not required.
