Perinatal Mental Health Case Management
The Perinatal Mental Health (PMH) Case Management program works within a multidisciplinary team through Oxford and Elgin County, providing rapid response and support to mothers who are pregnant and/or presenting with Post Partum Mood and Anxiety Disorders, for children up to the age of 24 months.
This program works towards ensuring the wellbeing of mother and child(ren) are paramount. We Aim to work alongside the individual seeking support to improve symptoms of mental health, quality of life, education, skill development, recovery plans and connection to ongoing supports and treatment options. This can be done through individual sessions as well as group supports where appropriate.
Life as a new or expecting mother can be difficult and overwhelming, we are here to support your care journey and look at ways to manage / understand the impacts of this new role.

What is Postpartum Depression (PPD)?
Postpartum Depression is an ‘umbrella term’ used to cover a variety of emotional
problems that can negatively affect the mother’s mood, outlook, behaviour etc, during pregnancy right after birth, or anytime within the first 2 years.
1 in 5 mothers will have depression or anxiety during pregnancy or postpartum.

Is this program for me?

Frequently Asked Questions
I am scared to reach out, what can I expect?
We will support you in a framework that best meets your needs, our case managers can meet in the community, at the office or at your home. If you are willing and commitment we are happy to go at a speed that eases engagement. You can expect warm, friendly caring professionals who are invested in your care journey. Sessions typically are about 60 minutes, however we know the gift of time may not be readily available as a new mom, therefore happy to adapt as needed. To better get to you know, we will ask a variety of screening and questions to better understand your treatment goals and care plan. Your feedback is important, if something is not working, please share this with your case manager.
Do children have to be in my care to engage in the program?
No, children may not be in the care of their parents, kinship and/or their primary caregiver and can still be involved with the program. If this is a goal of yours, this can explore this further in sessions and work collaboratively with community supports.
