Nurse Practitioner Service

The Nurse Practitioner Service provides innovative and comprehensive addiction and primary care services that support the health and well-being of clients in their recovery journey through inter-professional collaboration, respect and a commitment to excellence.
Primary health care is available through the NP led clinics for clients who do not have access to a family physician in the community. The goal of the clinic is to address the medical needs of individuals with addictions and mental health in conjunction with the care received from the psychiatrist and other team members. Coordinating ongoing care and having people participate more fully in their healthcare are two
important aspects of the clinic.

The goal of the Primary Care Clinic is to medically stabilize clients, improve overall well-being and transition them back to primary care close to their home for longer term support.

How Do I Get An Appointment?

Referral Process?
This program is available to any registered client of CMHA Thames Valley Addiction & Mental Health Services who is without primary care and experiencing barriers. Please note, this service is located in London and potential clients must have transportation available to attend appointments. 

Referrals can be made through the client’s existing support at CMHA.
This program does not accept walk-ins.

Who can make a referral?
A registered client of a CMHA program can be referred internally through their CMHA program staff.
