Transitional Case Management
Transitional Case Management (TCM) is a community-based service, short-term program designed for individuals struggling with mental health and/or addiction concerns who would benefit from support to address their immediate needs and to enhance overall mental health, safety and well-being. Transitional Case Management Services are voluntary and provided within a framework of psychosocial rehabilitation and recovery that promotes self-determination, hope and improved quality of life.
Goals & Objectives
This program is designed to help you navigate community supports, connect to resources and services and meet your short-term goals. A TCM might help you address housing issues, help transition back to community from hospital for example, budgeting, finding social and recreational supports and supporting you in making sure your physical and mental health needs are being addressed. The goal is to support the stabilization of clients in their natural environments by building healthy coping skills to live a fulfilling life.
Is This Program For me?
- Must be 16+ years
- Self-identified mental health or addiction need
- Must be willing to voluntarily engage
- Resides within London, Middlesex, Elgin or Exeter
- Expresses a desire to engage and work collaboratively to support ones goals
- Requires short term assistance with recovery goals
- Require system navigation and connection to community resources
How Do I Get An Appointment?
Referral Process:
Individuals can connect at anytime for TCM support either by Self-Referral or third party by clicking this link to start the process: https://cmhatv.ca/start/.
Who can make a referral?
The TCM program works directly with hospitals (LHSC and STEGH) and community partners for anyone with a mental health need looking for support upon their transition back to or within the community.
A referral can be made at any time while the person is within hospital.
